What the inspector wrote

On 12/12/2025 at 8:40am, Licensing Program Analyst (LPA) Jialing \342\200\234Julianne\342\200\235 Zhu, met with Licensee Azizeh Abdul Rahim for an unannounced Case Management visit for the purpose of a capacity increase . Present during the inspection were Licensee, fingerprint-cleared spouse, and two (2) children in care. The home was toured to conduct a Health and Safety Inspection in the on-limit areas only. The facility\342\200\231s hours of operation are Monday - Friday from 7:00am to 10:00pm. Application for Capacity Increase: All documents have been received for the capacity increase application. The fire clearance for a capacity of 14 children was granted by the Fremont Fire Department on 12/03/2025. Licensee was reminded to adhere to the restrictions of the fire clearance, which prohibits the garage to be used for day care purposes. LPA reviewed with Licensee the ratio and capacity requirements of a Large FCCH, including the requirement for the presence of an assistant at the facility. Whenever the assistant is not present, the licensee will comply with the capacity requirements for a Small Family Child Care Home (FCCH). LPA reviewed with Licensee staff file requirements for assistants. On-limit Areas (accessible by children in care): The on-limit areas are the living room (main daycare area), dining room, 1 st floor bathroom, kitchen, and backyard. The kitchen is only used as a walkthrough to the backyard. Off-limit Areas (not accessible by children in care): The off-limit areas are the entire 2 nd floor and garage. Page 1 of 2. See LIC 809C. LPA reviewed with Licensee the four (4) Advisory Notes issued on 10/01/2025 during the annual inspection to ensure Licensee is able to remain in compliance with Licensing regulations after capacity increase. LPA confirmed the issues identified in all Advisory Notes were corrected. Capacity Increase Approval: The home is recommended for an increase of capacity and will go effective today, 12/12/2025. Licensee rents the home. Licensee has not obtained a signed Property Owner/Landlord Consent form (LIC 9149). Without this consent, the facility can operate with a maximum capacity of 12 children when following the maximum capacity ratio. If property owner/landlord consent is obtained in the future, Licensee is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 14 children. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted, report was reviewed, and Appeal Rights were provided to licensee Azizeh Abdul Rahim. Page 2 of 2.
