What the inspector wrote

On August 12, 2025, at 1:50pm, Licensing Program Analyst (LPA) Randy Miranda met with Site Director Deval Mistry for an unannounced case management inspection. The purpose of the inspection was to discuss an unusual incident that occurred on 08/11/2025; which was properly reported to the Oakland South East Regional Office (OSERO) by the director on 08/12/2025. Present for the inspection were the site director, thirteen teachers, and forty-five children in care (ten-infants, fourteen 2-year-olds, three 2.5-year-olds, and eighteen 4-year-olds). During the case management inspection, LPA observed what appeared to be roach droppings and rodent droppings in the kitchen area, specifically under the kitchen sink cabinet. Other objects appeared to be insect eggs or dead larvae. Please see LIC809D. Regarding the Unusual Incident that occurred on 8/11/2025, at approximately 11:20am, the Dahlia Classroom was at the infant/toddler playground area. During this time, a new child\342\200\224who was visiting\342\200\224was integrated into the Dahlia Class to observe how they acclimated with the other children. When it was time to return indoors, staff members S2, S3, and S4 conducted a headcount and accounted for 14 children instead of the expected 15. The children were not visually confirmed. As a result, C1 was inadvertently left behind, possibly while playing or hiding in a play structure located at the far end of the infant/toddler playground. After the Dahlia Class had re-entered the facility, C1 was found by an adult not affiliated with the center, sitting in front of the infant/toddler playground door. The adult entered the play area through an unlocked gate and knocked on the door to alert staff. At that time, staff member S1 was in the girls\342\200\231 bathroom tending to a child when she heard the knock. S1 answered the door and was informed by the adult that a child (C1) was sitting at the doorstep. Absence of supervision is a Zero Tolerance violation. As a result, a Type A deficiency was issued during today\342\200\231s inspection. C alifornia Code of Regulations, Title 22, Division & Chapter Number 101229(a)(1) is being cited on the attached LIC 809D. As a result of the Type A deficiency cited during today\342\200\231s inspection, a copy of this report and LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to parents/guardians of current enrolled children in care and all children newly enrolled following a 12-month period of this report. LIC 9224 must be signed within the next business day the children are in care and are to be kept in the children\342\200\231s files. An immediate civil penalty of $500 is assessed today. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Exit interview conducted and report was reviewed with Site Director, Deval Mistry.
