What the inspector wrote

On October 6, 2025, at approximately 12:45 PM, Licensing Program Analyst (LPA) Elimika Woods conducted an unannounced Annual/Random Inspection and met with Director Anabel Avalos. LPA disclosed the purpose of the inspection and toured five (5) classrooms, both inside and outside, to conduct a Health and Safety inspection. During the inspection, there were thirty-six (36) preschool-age children present, along with six (6) staff members. The facility operates Monday through Friday from 7:45 AM to 5:45 PM. LPA reviewed the facility records, which showed that all individuals subject to criminal record review have obtained clearance or an exemption and are associated with the facility. A census was conducted, and the total number of children present matched the thirty-six (36) electronic sign-ins recorded in the system. At approximately 1:00 PM, LPA reviewed four (4) children’s files and two (2) staff files. All staff files contained the required Health Screening and Employee Rights forms. All children’s files contained the required documents, including Personal Rights, Medical Consent, and Identification & Emergency Information forms. At least one opening and closing staff member holds a current CPR and First Aid certification. At approximately 1:45 PM, LPA inspected the five (5) classrooms and observed teacher-to-child ratios to be in compliance. Classrooms contained adequate play and learning materials. Floor surfaces were well maintained, safe, and clean. Furniture was observed to be age-appropriate and in good condition. Classroom equipment was in good repair, and room temperatures were comfortable for children in care. Each child had individual storage space, and cubbies were labeled with children’s names to identify personal belongings. The designated isolation area for ill children is located in the director’s office, separate from the rest of the children in care. See 809-C. The facility is equipped with wired smoke and carbon monoxide detectors connected to a system that automatically notifies the fire department when activated. Per the director, the detectors are fully functional and serviced regularly. The center also has a working telephone, a pull-down fire alarm system, and multiple fully charged 2A10BC fire extinguishers, which meet the standards established by the State Fire Marshal. Disaster drills are conducted at least once every six (6) months, with the most recent drill conducted on 09/25/25. All required postings were observed in an area accessible to parents and the public. The outdoor play area includes a play structure with a slide and cushioned surfacing designed to absorb falls. The structure was securely anchored to the ground. Multiple trees and canopies provide adequate shade for children during outdoor activities. LPA was informed that staff bring children’s water bottles and cups outdoors to ensure they remain hydrated while playing. The play equipment was observed to be age-appropriate, well-maintained, and in good repair. The outdoor area was fully fenced and free from defects or hazardous conditions. A storage shed located on the playground was locked and inaccessible to children in care. Food Service, Sanitation, and Additional Observations This facility provides breakfast, lunch, and snacks for children in care. Monthly menus are posted at the facility. LPA observed the designated food preparation and storage area, which included a refrigerator. The area was clean, well-organized, and free from evidence of rodents or other pests. All food was properly stored and protected from contamination. Per staff, any contaminated food is discarded immediately. LPA reminded the director that all persons involved in food preparation and service must follow proper personal hygiene and food service sanitation practices to ensure food safety. The kitchen area is off-limits to children in care. All solid waste containers were observed to have tight-fitting lids and to be in good repair. LPA Observations During Visit- At approximately 1:00 PM, LPA observed all teachers positioned in their assigned areas supervising children during nap time, while some staff were on their lunch breaks. The nap-time environment was calm and soothing, with soft music and white noise playing in the background. Staff were spaced appropriately throughout the classrooms to maintain full visual supervision of all children. At approximately 2:25 PM, LPA inspected the staff bathroom and was informed by the director that it is separate from the children’s bathrooms. There are two (2) children’s bathrooms—one located in the main area of the center and another located inside Classrooms One and Two. LPA verified that all sinks and toilets were operating in a safe and sanitary condition by flushing toilets and running faucets to test water temperature. The water temperature was appropriate and not too hot for children in care. California Law requires Child Care Centers licensees to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. The director was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com . The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/ Lead Testing – Child Care Centers (CCC) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5 years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. The director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. The director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. There are no deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the director Anabel Avalos.
