, Licensing Program Analyst (LPA)
Elimika Woods
conducted an
unannounced Annual Random Inspection
and met with the director,
Rocio Guerrero
. LPA disclosed the purpose of the inspection, and a
health and safety inspection
was conducted inside and outside the facility. The center is located on the premises of a church and consists of two rooms for the infant/toddler component. During the inspection, there were 12 infant-age children present and four (4) additional staff members. The facility\342\200\231s hours of operation are Monday through Friday, 8:30 AM to 5:30 PM.
LPA reviewed the facility

, which indicat
e that

. LPA conducted a

of the children, which matched the

on the
sign-in sheet
. LPA reviewed
four (4) children\342\200\231s files
and
two (2) staff files
at approximately
8:55 AM
. All staff files contained the required
health screenings

Identification and Emergency Information
. At least one opening and one closing staff member have current
CPR and First Aid certifications

At approximately
12:15 PM
, LPA inspected the
two classrooms
and observed that
teacher-to-child ratios
were being maintained. There were adequate
play and learning materials
available. The
floor surfaces
were well maintained, safe, and clean, and the
furniture
was age-appropriate for infant children. Classroom equipment was in good repair, and the
room temperature
was comfortable for children in care. Each child had
individual storage space
with cubbies labeled with their names for personal belongings. The
isolation area
for ill children is located in the
director\342\200\231s office
, separate from other children in care.
The center is equipped with
wired smoke detectors
in each classroom, which are connected to the
fire department notification system
. According to the Director, the smoke detectors are fully functional and serviced regularly. LPA tested the
carbon monoxide detectors
and found them to be functioning properly. The facility has a
working telephone
, a
pull-down fire alarm system
, and multiple
fully charged 2A10BC fire extinguishers
throughout the center, meeting the standards established by the
State Fire Marshal
. Disaster drills are conducted at least once every six months, with the most recent drill conducted on October 9, 2025. All required postings were observed in an area accessible to parents and the public.

This facility provides breakfast and snacks for the children in care. Weekly menus are posted at the facility. LPA observed areas where refrigerators are located; these areas were clean and free of evidence of rodents. Food was properly protected from contamination and, per staff, any contaminated food is discarded immediately. LPA reminded the director that all persons engaged in food preparation and service must observe proper personal hygiene and food service sanitation practices to ensure food safety. All storage containers for solid waste were observed to have tight-fitting covers that are in good repair.
LPA\342\200\231s Observations During the Visit:
At approximately 12:00 PM, LPA observed the children during nap time, resting in their individual cribs while staff supervised. Some staff members were on their lunch breaks and returned before nap time concluded. Staff were positioned in the classroom with their backs against the wall to maximize visibility and supervision of all children.
The outdoor area for infants includes a play structure with a slide, which has cushioned surfacing to absorb falls. The outdoor play equipment is age-appropriate and in good repair. There are multiple trees providing shade for children while at play. The outdoor play area is fully fenced and free of defects or hazardous conditions. The playground is equipped with playhouses, tricycles, push cars, and toy vehicles, all of which help support the development of large motor skills.
LPA asked the Director if the staff bathroom was separate from the children\342\200\231s bathroom, and the Director confirmed that it is. The children\342\200\231s bathroom is located to the right of the Director\342\200\231s office. LPA checked and flushed all toilets in the children\342\200\231s restroom to verify they were in working order. LPA also tested the water temperature at the sinks to ensure it did not get too hot for children. All sinks, toilets, and faucets were found to be in safe and sanitary operating condition during today\342\200\231s inspection.
California Law requires Child Care Centers licensees to
report unusual incidents
or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. The director was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting

Lead Testing \342\200\223 Child Care Centers (CCC) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

The director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
The director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
There are no deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the director, Rocio Guerrero.
