What the inspector wrote

On December 11, 2025, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux arrived and conducted an unannounced Annual Random 1 year Required Inspection and met with Director/owner, Li Guan . LPA disclosed the purpose of the inspection and was granted entry into the facility. There were sixty-nine (69) children present during this inspection and 12 additional staff members (includes administrative staff). The facility was toured for a health and safety inspection. Hours of operation is 8:00 AM-6:00 PM, Monday - Friday . This program starts with children at age of 2 years old. (no infants) CLASSROOMS: Five (5) classrooms was inspected. There are play and learning materials available, floors, furniture, and equipment are age appropriate and in good repair. There is adequate heating/air conditioning, ventilation and lighting. Drinking water is available inside and outside. There is proper individual storage space for each child. The center has a wired smoke detectors, a carbon monoxide detector in the hallway (tested and functioning), working telephone, and pull down fire alarm system, and fully charged 3A40BC fire extinguishers throughout the facility. BATHROOMS AND TOILETING AREAS: Staff's bathroom is separate from the children's bathroom. All sinks and faucets are in safe and sanitary operating condition. The children are able to reach the sinks and toilets. Supplies are available to the children. FOOD SERVICE AREA: This facility does not provide lunch, lunch is supplied by parents. This facility provides AM/PM snacks for the children. Monthly menu is posted. Food items for snacks is stored in the staff luncheon area. OUTDOOR PLAY AREAS: The school has a fully fenced playground area that the facility use for play and all equipment is in good repair. There's a small play structure that is anchored with cushioning to absorb children's falls. There are no bodies of water, or free-standing water accessible to children. Director confirmed there are no pools/bodies of water at the facility. There are age appropriate toys and materials for the children. RECORDS: LPA requested files and reviewed children and staff files at approximately 10:30 AM. All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. LPA reviewed and qualified staff. Six (6) Children's files and staff files were reviewed. All staff files have required Health Screening and Employee Rights. All children's files contained Identification & Emergency, Personal Rights, and Medical Consent forms. LPA reviewed the facility roster and obtained a copy. Director's has a current CPR & First Aid Certificate expires 12/6/27 and Mandated Reporter Training completed on 11/19/25. The center is in compliance with the sign in and out procedure. Disaster drills are being conducted at least once every 6 months at the last one conducted was on 12/04/2025. All required documents are posted in a public accessible area. Criminal Record Clearance : Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Lead Testing: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPA noted the Lead Report on the parent board. See 809-C Incidental Medical Services (IMS): This facility does provide Incidental Medical Services (IMS). LPA inspected storage of medications and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Unusual Incident/Student Injury Report: Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child\342\200\231s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days. MyChildCarePlan.org: Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
