What the inspector wrote

On 04/08/2024 at 9:11am, Licensing Program Analysts (LPAs) Christina Uribe & Morgan Pringle conducted an unannounced Case Management visit and met with Director, Aziza Brumfield. Also present at the time of the inspection is 3 staff members and 26 children. The purpose of today's visit was to investigate a complaint and during the course of the investigation, LPAs found deficiencies unrelated to the complaint resulting in this case management report. At around 10:00am, LPA Uribe toured the facility for a secondary count of children present in care. While in the Toddler classroom, the director informed LPA Uribe that two staff members had taken the preschool class on a walking field trip with 15 children. During record review, LPAs determined that the two staff members that are with the preschoolers on the walking field trip are both unqualified and considered aides. Due to the classroom being out of ratio away from the facility and aides supervising children without the direct supervision of a fully qualified teacher, two Type B Violations are being cited today. Please see deficiency pages LIC 809D for additional information and plan of correction. Plan of correction is due on 04/11/2024. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeals rights were given and reviewed. Exit interview was conducted and report was reviewed with the director, Aziza Brumfield.

    101216(h) Personnel that pose a threat to the health and safety of children shall be relieved of their duties. This requiremnt was not met as evidenced by, through LPAs investigation it was found that facility staff were required to work at the facility with the children when they were ill. This presents a potential risk to the health and saftey of the children in care.

  • PERSONAL RIGHTS

    101223(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain,...or other actions of a punitive nature...This requirement was not met as evidenced by Through LPAs investigation it was found that child(ren) in care had been hit by facility staff. This presents a potential risk to the health and safety of the children in care.

  • TEACHER-CHILD RATIO

    101216.3: Teacher-Child Ratio (a) There shall be a ratio of one teacher visiually observing and supervising no more than 12 children in attendance.This requirement was not met as evidenced by: During today's visit, LPAs observed two aides supervising 15 children alone without the direct supervision of a fully qualified teacher which poses a potential risk to the health, safety, or personal rights to children in care.

    101216.3: Teacher-Child Ratio (a)(2) Whenever children are engaged in activities away from the center, no teacher shall be in charge of a group of more tha 12 children.This requirement was not met as evidenced by: During today's visit, LPAs observed two aides supervising 15 children alone without the direct supervision of a fully qualified teacher on a walking field trip away from the center which poses a potential risk to the health, safety, or personal rigths to children in care.
