What the inspector wrote

On April 8th, 2025 at, 9:30 AM Licensing Program Analyst (LPA) Michael Mathew conducted an unannounced inspection to conclude a complaint investigation LPA met with Director Dawn Gaddini and advised her the purpose of the inspection. Director and LPA toured the facility inside and out. There were 27 children and 4 staff in care at the time of the inspection. LPA interviewed staff members, reporting party, children, and parents. Based on interviews conducted, during a field trip that was on 4/2/2025 to Chuck E. Cheese the facility received a phone call that they left two children at Chuck E. Cheese Hayward. therefore the preponderance of evidence standard has been met, therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Article 06, Section: 101229(a)(1) Responsibility for Providing Care and Supervision, are being cited on the attached LIC 9099D. This is a Zero Tolerance Violation and an immediate $500 civil penalty is assessed today. A notice of site visit and appeal rights were given and must be posted for 30 days. Exit interview conducted and report was reviewed with director Dawn Gaddini Upon receipt of licensing report, licensee must provide a copy of this report to parents/guardian of all children enrolled and any newly enrolled parents/guardian for 12 months from the date of this report and a signed Acknowledgement of Receipt of Licensing Report must be placed in the children's files.
