What the inspector wrote

**This is an amended report to remove the deficiency that was cited for reporting requirements. This deficiency was dismissed via appeal. No other changes were made.** On April 8th, 2025 at 9:30am, Licensing Program Analyst (LPA) Michael Mathew conducted a case management inspection while at the facility for another reason. LPA met with director Dawn Gaddini. There 27 children and an additional 5 staff members present. An unusual incident occurred during a field trip on 4/2/2025 which resulted in 2 children were left behind at a Chuck E Cheese. No deficiencies were cited. Exit interview conducted with director Dawn Gaddini and copy of report provided. Notice of Site Visit provided and must be posted for 30 days.
