On 05/21/2025, Licensing Program Analysts (LPAs) Jyoti Saini and Kassandra Medrano conducted an unannounced case management inspection regarding an Unusual Incident that was self-reported by the facility on 05/13/2025. LPAs met with the assistant director, Lauren Oh and conducted staff interviews, reviewed records, and made facility observations. Present during the inspection were the assistant director and nine (9) staff members supervising 56 preschool aged children. Based off the provided footage an incident occurred on 05/09/2025. LPA\342\200\231s determined that on the date mentioned, children were observed interacting with one another on multiple instances. However, Staff interviews revealed that although the interactions were documented by footage, they were not observed by staff present. The facility was advised to submit a written statement outlining their plan to prevent this incident from happening again. No citations were issued. A notice of site visit was posted and must remain posted for 30 days. An exit interview was conducted with Assistant Director, Lauren Oh.
