What the inspector wrote

On April 17, 2025, License Program Analyst (LPA) arrived unannounced to conduct an Case Management- Incident Inspection. LPA Dacanay Breaux informed the director the nature of the visit and was granted entry by facility representative, Janani Krishnakumaar. This inspection is due to the facility self- reported an Unusual Incident Report (UIR) dated 04/14/2025. Facility is dual licensed and holds a license for infant component #015700532. During today's visit there were 9 teachers and 2 administrative staff, 36 preschool age children in care. Upon arriving LPA Dacanay Breaux toured the facility. LPA conducted interviews with the center director and owner and obtained documents. LPA previewed the video. Based on the interviews and video, the facility acknowledged that " a staff member did indeed hit a child". As a result, a Type A deficiency is issued during today\342\200\231s inspection. C alifornia Code of Regulations, Title 22, Division & Chapter Number 101223(a)(3)} is cited on the attached LIC 809D. As a result of the Type A deficiency cited during today\342\200\231s inspection, a copy of this report and LIC 9224 Acknowledgment of Receipt of Licensing Reports must be provided to parents/guardians of current enrolled children in care and all children newly enrolled following a 12 - month period of this report. LIC 9224 must be signed within the next business day the children are in care and is to be kept in the children\342\200\231s files. An exit interview was conducted with facility representative/owner, Janani Krishnakumaar. A notice of site visit was given and must remain posted for a period of 30 days. Appeal Rights Provided.

This requirement was not met as evidenced by: A child in care was hit with a book by a former teacher which poses an immediate risk to the health and safety of the children in care.
