On May 21, 2025 at 12:17pm, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux conducted an unannounced case management visit for the purpose of investigating a reported incident dated 04/13/2025. LPA met with facility representative/owner, Janani Krishnakumaar . Also present during the inspection were nine (9) fingerprint -cleared staff and 55 children in care. This facility has an infant component #015700532. Today's visit was a follow up visit due to an incident that occurred on 04/13/2025 which was self - reported to Licensing on 04/14/2025. Per director, one (1) child was hit by teacher on 4/11/2025. LPA reviewed records and interviewed staff and children about the incident. A Notice of Site Visit was given and must remain posted for 30 days. Appeal rights provided. Exit interview was conducted, report was reviewed with facility representative, Janani Krishnakumaar.
