What the inspector wrote
On May 20, 2026, at 11:30 AM, Licensing Program Analyst (LPA) Elimika Woods conducted an unannounced annual random inspection and met with Director Jamila Mohamed. LPA disclosed the purpose of the inspection and conducted a health and safety inspection of the facility, including both indoor and outdoor areas. The facility operates as an infant center with a licensed capacity of 16 children and is open Monday through Friday from 8:00 AM to 5:00 PM. At the time of the inspection, four infants were present and supervised by two fully qualified teachers. All individuals subject to criminal record review have received a criminal record clearance or exemption and are associated to the facility.
LPA conducted a census of the children in care and verified that the census matched the four electronic sign-in signatures. At approximately 11:45 AM, LPA inspected the facility’s two classrooms and observed that the required teacher-to-child ratio was being maintained. The classrooms were organized to allow infants to explore different activity areas. Adequate play and learning materials were available, and all furniture and equipment were age-appropriate and in good repair. The floors were clean, and each child had an individual storage space for personal belongings. The designated isolation area for ill children is the Director’s office, located away from other children in care.
At approximately 11:45 AM, LPA reviewed two children’s files. Each file contained the required documentation, including Personal Rights, Parent’s Rights, Medical Consent forms, and Identification and Emergency Information. The facility conducts disaster drills at least once every six months. The last documented drill was conducted on May 6, 2026.
At approximately 12:45 PM, LPA reviewed two staff files. Each file contained the required Health Screening form, Employee Rights documentation, and the Statement Acknowledging Requirement to Report Child Abuse. At least one staff member present during opening and closing hours holds a current Pediatric CPR and First Aid certification.
LPA tested the facility’s smoke and carbon monoxide detectors and found them to be functioning properly. The center is equipped with emergency backpacks and multiple fully charged 2A10BC and 3A40BC fire extinguishers, which meet the standards established by the State Fire Marshal. All required licensing documents were posted in a location accessible to parents and visitors.

The facility provides breakfast, lunch, and snacks for the children in care, and weekly menus are posted and accessible at the facility. LPA reminded the director that all individuals involved in food preparation and service must follow proper personal hygiene and food sanitation practices to prevent food contamination. LPA also observed that all solid waste containers are equipped with tight-fitting lids that are in good repair.
LPA Observations During the Visit:
At approximately 12:30 PM, children were observed sitting at tables with teachers while eating lunch. One staff member began preparing sleeping equipment in anticipation of nap time. During nap time, staff played soft, calming music to support children’s rest. The classroom lights were turned off, and staff maintained supervision of the children from various areas within the room to ensure ongoing monitoring and safety.

LPA observed that the staff’s bathroom was separate from the children's bathroom. The children's bathroom are located inside the classrooms. each bathroom has a changing table that's in arms reach of a faucet. LPA verified the toilets were working properly by flushing them to make sure it was operating in a safe condition and that the water did not get to hot for children in care. The center has plenty of soap, bath tissue, diapers, and paper towels available to children in care.
The playground area is fully fenced and free from defects and dangerous conditions during todays inspection. The playground has canopies that provide shade to children while at play. The play structure is anchored into the ground with cushioning underneath to absorb falls. The play equipment is age appropriate and in good repair. California Law requires Child Care Centers licensees to
report unusual incidents
or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. The director was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting

Lead Testing – Child Care Centers (CCC) CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

The director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
The director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
There are no deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the director
