What the inspector wrote
On January 12, 2024, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux arrived announced for a Pre licensing Inspection. LPA met with applicant Deepthi Koduru and her husband Vishnuchaitanya Panta. Also residing in the home is the applicant’s two children (4 year old son, 11 year old daughter). All adults have proper fingerprint and TB clearances. The home was toured with the applicant to conduct a health and safety inspection. Applicant states the hours of operation for day care will be Monday through Friday, 8:00 am to 5:30pm.
The home is a two story single family home 3 bedroom and 2.5 bathrooms. First floor which consists of a living Room, family room, kitchen, half bathroom and two car garage. Second floor master bedroom and bathroom, full bathroom, loft area and 3 bedrooms (includes master/bath). There is also a fully fence rear backyard. The home is neat and clean with heating and ventilation for safety and comfort.
ON LIMITS
:
First floor livingroom, family room, kitchen and half bathroom and rear yard.
The
isolation area
will be in the first livingroom away from the children in care.
OFF LIMITS
:
Entire Second Floor, three bedrooms (including masterbedroom and bathroom) full bathroom, and the loft area. The garage on the main level. Rear yard, right side there is a safety gate to keep the children out of the area, and on the left side small section off limits, during todays inspection there is a safety gate. There is shed in the off limits with a lock. All Off Limit areas will be inaccessible by closed and/or locked doors and visual supervision. The applicant was advised to contact Licensing, so that an inspection can be completed prior to changing an off limits area to on limits.
The outdoor play area will be the backyard, which has a fence surrounding the perimeter of yard, and is free from defects and/or dangerous conditions. There are ample age appropriate toys observed to be safe, clean and in good repair. There are no pools, hot tubs or any other bodies of water. LPA did not observe any hazardous materials or toxins accessible to children today. The home has a fully charged 2A10BC fire extinguisher which is located in the first floor in the garage, working carbon monoxide and smoke detector throughout the home (tested and functioning), telephone, and fully stocked first aid kit. Applicant has installed child proof gates at the bottom of stairs to prevent access by children. Applicant is aware the gates must always be secure during day care hours. The fireplace is screened and location in the family room across from the kitchen. Per applicant, there are no firearms and no pets in the home during today's inspection.
The applicant’s health and safety training has been completed, and includes the child nutrition and lead poisoning components. EMSA approved First Aid/CPR certificate is current, expiring on
02/04/2025.
A copy of the lease agreement has been reviewed and shows control of property. The applicant has provided proof of the required immunization's for daycare providers, and mandated reporter training was completed on
12/29/2022

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
Because the applicant leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).

LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.
LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at:

, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
The California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex against all CCLD facility addresses pursuant to information shared by California DOJ.
Applicant was informed of the
MyChildCarePlan.org
site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Child Care Centers and Family Child Care Homes: Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
