What the inspector wrote

On April 29, 2026 at 4:05pm, Licensing Program Analyst (LPA) Julia Placencia arrived unannounced for a plan of correction (POC) inspection to confirm corrections have been made for deficiencies cited during an Annual/Random Inspection conducted on 4/27/2026. LPA met with licensee Caixia Li. There were eleven children and one helper present today. Section 102416.5(d)(1) Staffing Ratio and Capacity was cited having five infants and seven preschool age children present making the facility out of ratio. LPA observed today there were three infants and eight preschool age children. Facility is within its ratio/capacity limits today. Per licensee, one infant has disenrolled. LPA observed signed Acknowledgement of Receipt of Licensing Reports forms (LIC 9224) for children enrolled. The deficiency is cleared today. Section 1596.8662(b)(1) Administration of Child Day Care Licensing was cited for licensee and helper not having current mandated reporter certificates. LPA observed today that licensee and helper have completed mandated reporter training. The deficiency is cleared today. Section 1597.622(a)(1) General Provisions and Definitions was cited for helper not having proof of measles immunization. LPA observed today helper has received the immunization. The deficiency is cleared today. No other deficiencies cited today. Copy of Letter of Deficiency Citations Cleared provided at visit. Exit interview conducted with licensee Caixia Li. Notice of site visit was provided and must remain posted for 30 days.
