What the inspector wrote

On 5/9/25, Licensing Program Analyst Briana Plumboy, met with licensees Zahra Hosseinmighadam & Narges Kolmer for an UNANNOUNCED ANNUAL INSPECTION. Present for this visit was 3 infants, 2 preschool age children, and Gholamhossein Hosseinmoghadam (Zahra's father & Narges husband) whom is fingerprint cleared and associated. The home was toured to conduct a Health and Safety Inspection. At this time there are no children residing in the home. The licensee's operate the facility 7 days a week from 5:00am until 10:00pm. The home is single story. The home consists of a living room, hallway bathroom, 3 bedrooms, kitchen/dining area, master bedroom with bathroom, and ADU unit located in the backyard (not permitted by the city). The home appears to be neat and clean with heating and ventilation for safety and comfort during today's inspection. The OFF LIMIT AREAS are the 2 bedrooms (bedrooms #2 & #3), kitchen area, backyard, and ADU unit which will be inaccessible by closed doors and visual supervision. As of 5/9/25, the master bedroom with bathroom (located off the kitchen) and dining area are included in the on limit areas. The ON LIMIT AREAS are the dining area, the master bedroom with bathroom (located off the kitchen), the living room, hallway bathroom, frontyard, and bedroom #1 (first bedroom on right side of hallway). The ISOLATION AREA will be the dining area. Licensees are aware when children play in the front yard there must be 100% physical and visual supervision on the children in care at all times. The FRONTYARD play area is fenced. There are toys and learning materials that appear to be safe and in good condition. There are no pools, hot tubs or any other bodies of water present in the on limit areas during today's inspection. All hazardous materials and toxins are kept out of the reach of children and it was observed that during the inspection there are no toxins or hazardous items accessible. See 809-C for continuance The home has a fully charged 2A10BC fire extinguisher, working smoke detector, working carbon monoxide detector, and working telephone. Both licensee’s Health and Safety training are completed and CPR and First Aid certificates are current. Zahra Hosseinmoghadam's CPR/First Aid certificate expires 2/5/26 & Narges Kolmer's CPR/First Aid certificate expires 2/7/26. The licensee's completed and received certificates in mandated reporter training both on 5/16/24. The licensee's are in compliance with immunization law which pertains to day care providers. The floor heaters are screened to prevent access by children. Per licensee's, there are no firearms in the home. The licensee conducts and documents fire and disaster drills twice a year with the last one conducted on 4/8/25. (5) Children files were reviewed, facility roster reviewed and copy obtained. The licensee is in ratio today. All REQUIRED forms are posted and visible for public review. Licensees are aware they should have knowledge of all Title 22 Regulations and follow all Title 22 Regulations at all times, as well as follow manufacture guidelines for all equipment in the facility. The licensee's are reminded any structural changes to the home or additions to the child care facility must be reported to Community Care Licensing. Also, any adults moving into the home must be reported to Community Care Licensing prior to them moving in and all requirements must be met before the person lives in the facility. Licensees were reminded of Departments inspection authority, with our without any notice. California Law requires Family Child Care Home licensees to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. Roster of the children must be properly maintained and fire/disaster drill every six months must be documented. The licensees were also reminded that baby bouncers, exersaucers, johnny jumpers and similar items are not allowed in licensed day care. Licensees were reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. See 809-C for continuance Licensees are encouraged to frequently visit our website at ccld.ca.gov for licensing regulations and updates. Licensees were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-carelicensing/subscribe and select the Child Care option to receive email communication. LPA discussed the safe sleep regulations with licensees and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed licensee licensees of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. During the exit interview, the Licensees confirmed that there are no Registered Sex Offenders living in the facility. This entire report has been read to the Licensee by LPA Plumboy. The licensees are aware the signature on this report confirm receipt of these documents. LPA asked the licensees if the licensees had any questions pertaining to any aspects including, but not limited to, any part of this report and of the documents given to the licensees, and per licensees, there are no further questions at this time. Licensees are aware at anytime they can reach out to LPA Plumboy or CCLD. A notice of site visit was given and must remain posted for 30 days. No deficiencies today's inspection. Appeal rights provided and discussed. Exit interview conducted and report was reviewed with licensees.
