What the inspector wrote

On 5/5/26, Licensing Program Analyst Briana Plumboy, met with licensee Zahra Hosseinmighadam for an UNANNOUNCED ANNUAL INSPECTION. Present for this visit was 4 infants, 2 preschool age children, S2 (licensees assistant/ mother) and S1 (licensees assistant/ father) whom are fingerprint cleared and associated. The home was toured to conduct a Health and Safety Inspection. At this time there are no children residing in the home. The licensee operates the facility 7 days a week from 5:00am until 10:00pm. Approved 850 comments, "Fire Clearance Granted. *Note Update Facility Sketch reflecting living room, all bedrooms, bathroom #1, and kitchen is off limits to children*" CHILDREN IN CARE MAY ONLY UTILIZE THE DAYCARE ROOM & BATHROOM IN THE DAYCARE ROOM WHICH IS THE ROOM LOCATED IN THE BACK OF THE HOME OFF THE DINING ROOM AREA, AND THE DINING ROOM AREA ONLY. THE BARRIER SHOULD BE IN PLACE WHICH PREVENTS ACCESS TO THE KITCHEN AND REMAINDER OF THE HOME WHICH IS OFF LIMITS TO CHILDREN IN CARE AT ALL TIMES. The home is single story. The home consists of a living room, hallway bathroom, 3 bedrooms, kitchen/dining area, master bedroom with bathroom, and ADU unit located in the backyard (permitted by the city with a separate address. The home appears to be neat and clean with heating and ventilation for safety and comfort during today's inspection. The OFF LIMIT AREAS are ALL bedrooms, hallway bathroom, kitchen area, living room, backyard, and ADU unit which will be inaccessible by closed doors and visual supervision. The ON LIMIT AREAS are the dining area, daycare room, bathroom in the daycare room, and frontyard. The ISOLATION AREA will be the dining area. Licensee is aware when children play in the front yard there must be 100% physical and visual supervision on the children in care at all times. The FRONTYARD play area is fenced. There are toys and learning materials that appear to be safe and in good condition. There are no pools, hot tubs or any other bodies of water present in the on limit areas during today's inspection. All hazardous materials and toxins are kept out of the reach of children and it was observed that during the inspection there are no toxins or hazardous items accessible. See 809-C for continuance The home has a fully charged 2A10BC fire extinguisher, working smoke detector, working carbon monoxide detector, and working telephone. The licensee and S2 completed and received certificates in mandated reporter training on 5/16/24, and S1 completed his mandated reporter training on 4/28/26. The licensee, S1, and S2 are in compliance with immunization law which pertains to day care providers. The floor heaters are screened to prevent access by children. Per licensee there are no firearms in the home. Licensee is utilizing LIC 9227 and documenting 15 minute sleep checks. The licensee conducts and documents fire and disaster drills twice a year with the last one conducted on 4/8/26. Children files were reviewed. Facility roster reviewed and copy obtained. All REQUIRED forms are posted and visible for public review. Licensee is aware she should have knowledge of all Title 22 Regulations and follow all Title 22 Regulations at all times, as well as follow manufacture guidelines for all equipment in the facility. The licensee is reminded any structural changes to the home or additions to the child care facility must be reported to Community Care Licensing. Also, any adults moving into the home must be reported to Community Care Licensing prior to them moving in and all requirements must be met before the person lives in the facility. Licensee was reminded of Departments inspection authority, with our without any notice. California Law requires Family Child Care Home licensees to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. Roster of the children must be properly maintained and fire/disaster drill every six months must be documented. The licensee was also reminded that baby bouncers, exersaucers, johnny jumpers and similar items are not allowed in licensed day care. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. See 809-C for continuance Licensee was encouraged to frequently visit our website at ccld.ca.gov for licensing regulations and updates. Licensees were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. During the exit interview, the Licensees confirmed that there are no Registered Sex Offenders living in the facility. This entire report has been read to the Licensee by LPA Plumboy. The licensee is aware the signature on this report confirm receipt of these documents. LPA asked the licensee if the licensee had any questions pertaining to any aspects including, but not limited to, any part of this report and of the documents given to the licensee, and per licensee, there are no further questions at this time. Licensee is aware at anytime they can reach out to LPA Plumboy or CCLD. A notice of site visit was given and must remain posted for 30 days. There were no deficiencies cited during today's inspection. Appeal rights provided and discussed. Exit interview conducted and report was reviewed with licensee Zahra Hosseinmighadam.
