What the inspector wrote

On 07/02/2025 Licensing Program Analysts (LPAs) Jaleesa Jackson and Jialing (Julianne) Zhu conducted an unannounced case management visit. LPAs met with Licensee Ruijie (Annie) Li. Present for the inspection were her 2 fingerprint cleared assistants, 4 infants and 8 preschool age children. During a complaint investigation conducted by Investigator Victoria McIntosh it was found that Licensee\342\200\231s assistant Cathy Wu (legal name unknown at this time) was present in the facility on 2/14/2025 and did not have an eligible criminal record clearance for the 3 days that she was present in the facility. Through interviews it was also found that a second assistant Na "Hanna" Wu was present and working in the facility part time in the beginning of the year. While both unclear adults no longer work at the facility they were still present around the children without a criminal record clearance. The Licensee reported on 2/14/2025 that C1 sustained a fracture while in care but was made aware of this by parents on 2/12/2025 that C1 was taken for medical treatment. The Licensee did not inform the Regional Office by the next business day that a child in her care had an injury that required medical treatment. It was also found that Licensee did not have a file for C2 during a visit on 5/20/2025 during Investigator McIntosh's visit. Licensee stated at the time that C2 had just started care that morning. As of today's visit C2 has a completed file that was filled out 5/22/2025. There were 3 deficiencies cited. See 809-D for deficiencies cited during today's inspection. Continued on 809-D LPA Jackson informed Licensee Ruijie Li that this report dated 07/02/2025, documenting one Type A deficiency, shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. LPA Jackson informed the licensee to provide a copy of this licensing report, dated 07/02/2025 that documents a Type A deficiency, to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted with Ruijie Li and appeal rights provided.

This posed an immediate health, safety or personal rights risk to persons in care.

    Prior to employment or initial presence in the child care home, all employees and volunteers subject to a criminal record review shall: Obtain a California clearance or a criminal record exemption as required by law or Department regulations.This requirement is not met as evidenced by: Based on record review and interview Cathy Wu was working in Licensee’s daycare and present around the children for 3 days without a fingerprint clearance which posed an immediate health, safety or personal rights risk to persons in care.

    An emergency information card shall be maintained for each child and shall include the child's full name, telephone number and location of a parent or other responsible adult to be contacted in an emergency, the name and telephone number of the child's physician and the parent's authorization for the licensee or registrant to consent to emergency medical care. This requirement is not met as evidenced by:Based on record review, Licensee was not able to provide a file for C2 during Investigator McIntoish’s visit on 5/20/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
