What the inspector wrote

On December 11, 2025 at 12:25PM LPA Dacanay Breaux arrived to conduct an Case Management Inspection. Present for this inspection was licensee, assistant and 5 children in care (2 infants and 3 preschool age children). During record review for the case management inspection, LPA found that current staff (assistant) did not have an eligible background criminal record/fingerprint clearance. LPA contacted regional office and the application was incomplete. Assistant has been working for licensee since March/April, 2024 (per licensee). The issuance of a Type A Violation is being issued today and the assessment of an immediate $500.00 civil penalty. See LIC9099-D. LPA Dacanay Breaux informed the licensee to provide a copy of this licensing report dated 12/11/2025 that documents the Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care , and to any newly enrolled parents/guardians for 12 months from the date of this report . A signed Acknowledgement of Receipt of Licensing Report (LIC 9224) , must be placed in the child's file for verification. LPA provide a copy of the LIC9224 to licensee. Notice of Site Visit and Appeal Rights were given. Notice of Site Visit must be posted for 30 days. Failure to comply with posting requirements will result in the issuance of an immediate $100 civil penalty. Exit interview conducted and report was reviewed with licensee, Kailan Miner.

    101170 - Criminal Record Clearance: (e) All individuals subject to a criminal record review shall prior to working, residing, or volunteering in a licensed facility; (1) Obtain a criminal record clearance or a criminal record exemption as required by the Department. During case management inspection, a staff was working at the FCCH. S1 did not have an eligible criminal background clearance on file. This requirement was not met as evidenced by: S1 did not have eligible clearance which is an immediate health, safety, and personal rights risk to children in care.
