What the inspector wrote

On 02/26/2026, at 1:00pm, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced Case Management Visit and met with licensee, Kalaisri Sankar Kumar. Also present at the time of today's visit is the licensee's fingerprint cleared husband and 3 day care children. The purpose of today's visit is to inspect the home for the increase of capacity request initiated by the licensee. Application for Capacity Increase: All documents have been received for the increase of capacity application. The fire clearance for a capacity of 14 children was received from the Newark Fire Department on 02/20/26. The licensee was reminded to abide by the conditions of the fire clearance. The licensee is reminded that prior to accepting more than 8 children into care, an assistant must be hired, fingerprint cleared, and associated to the facility. The licensee was reminded that the assistant must be present at all times when there is more than 8 children in attendance. Whenever the assistant is not present, the licensee will comply with the capacity requirements for a small family child care home. Assistant Requirements: All assistants must obtain the following: 1) Criminal Background Clearance; 2) Mandated Reporter Certification; 3) Proof of TB Clearance; 4) Proof of Immunization against Measles (MMR) & Pertussis (Tdap); 5) A signed Statement Acknowledging Requirement to Report Child Abuse (LIC 9108) form; 6) A signed Employee Rights (LIC 9052) form. The application for an increase of capacity to a Large Family Child Care Home License is approved and shall go into effective as of this date, 02/26/2026. A notice of site visit was given and report was reviewed with the licensee, Kalaisri Sankar Kumar. Page 1 of 1 ***End of Report***
