What the inspector wrote

Licensing Program Analyst (LPA) Loraine Perez met with Facility Representative, Hannah Bonano, for the purpose of conducting an unannounced complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained to Director During today's inspection, LPA conducted interviews, observed care, and obtained relevant documentation. Witness statements, and document reviews failed to corroborate the allegations. Center has implemented changes to the program to ensure the safety of children in care. It was revealed in interviews that requests for more staff training has been made to manage challenging behaviors. Although the allegation may have happened, there is not a preponderance of evidence to prove the allegation; therefore, the allegation is unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Hannah Bonano. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
