Inspector’s narrative

What the inspector wrote

It was alleged that staff did not notify day care child's responsible party of incidents. There was a concern that the facility did not contact C1's authorized representatives of incidents caused by C1. LPA learned through record review and staff interviews that parents of children who obtain serious injuries are called immediately. When children obtain less serious injuries, a report is sent via Brightwheel to the injured child's authorized representative or a written report is provided during pick up. Parents interviewed did not have concerns regarding reporting of incidents and state that any injuries are communicated to them right away or during pick up. Interviews with staff, children, and parents of children at the facility did not support the allegations. Based on the information obtained throughout the course of this investigation for the above allegations, LPA Tedlos determined that the allegations were found to be UNSUBSTANTIATED, meaning although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. An exit Interview was conducted with the Director, Kirbie Allen. A notice of site visit was provided and must remain posted for 30 days ***This is an amended version of an original report dated 05/12/26.***
