What the inspector wrote

On 7/9/25 @ 11:06am an unannounced case management inspection was conducted by Licensing Program Analyst (LPA), Erica Laird. LPA met with superintendent Gary Rogers in response to an Unusual Incident Report received by the Department on 6/28/25. Per the incident report, on 6/27/25 a child (C1) was left alone in a classroom for approximately 15-20 minutes. On 7/9/25 @ 11:16am LPA Laird conducted an interview with superintendent Gary Rogers. Gary stated he was aware of the incident . Gary stated staff had been interviewed and video footage from the classroom was reviewed. Gary stated, per the video footage, C1 was left alone in the classroom for 30 minutes. Gary stated the child was not observed leaving the classroom at any time. On 7/9/25 LPA Laird reviewed video footage from the incident and confirmed C1 was left alone in the classroom for approximately 30 minutes. On 6/28/25, 7/3/25 and 7/9/25 LPA Laird conducted interviews with two staff (S1, S2) and one parent (P1). All those interviewed stated they were aware of the incident. All those interviewed stated C1 was left alone in the classroom for approximately 15-30 minutes. Once staff were notified of C1's absence, office staff immediately went to the classroom and found C1. C1 was upset and crying but calmed down quickly. report continued on 809C During today\342\200\231s inspection, the facility was toured . LPA Laird took 5 photos and obtained a summary of events from superintendent Gary Rogers. Based on interviews and video footage the following deficiency(ies) is being cited on the LIC809-D. 1596.99(c)(3) LPA Erica Laird informed Gary Rogers that this report dated 7/9/25 documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. LPA Erica Laird informed superintendent Gary Rogers to provide a copy of this licensing report dated 7/9/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with superintendent Gary Rogers. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
