What the inspector wrote

An unannounced case management inspection was conducted today at 2:20pm by Licensing Program Analyst (LPA), Erica Laird. LPA met with facility director Jackie Hansen in response to an Unusual Incident Report received by the Department on 9/24/25. Per incident report, on 9/23/25 C1 scratched C2 on two separate occasions drawing blood. On 10/14/25 LPA Laird conducted an interview with director Jackie Hansen. Jackie stated the incident happened on 9/23/25 @ approx. 3:15pm in the "Magnolia" room. Jackie stated C1 scratched C2 on two separate occasions during the day. Jackie stated staff were present at the time of both incidents. Jackie stated she was not present during the incident but was told C1 acted out aggressively towards C2 without warning. Jackie stated C1 scratched another child (C3) yesterday. Jackie stated she called the incident into licensing on 10/13/25. On 10/14/25 LPA Laird conducted three staff interviews (S1-S3). All staff interviewed stated staff were near C1 when they scratched the other children in care. All staff stated there were no warnings prior to C1 scratching the other children. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with facility director Jackie Hansen. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
