What the inspector wrote

An unannounced case management inspection was conducted today at 2:14pm by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with facility representative Amy Beth Ahl-Wright in response to an Unusual Incident Report received by the Department on 9/25/25. A child in care, (C1) fell on the play structure and hit their forehead causing a cut which required stitches. The facility representative was interviewed on 9/29/25 at 2:14pm and stated that she was not present on the playground during the incident. She was informed that the incident occurred and she believed that C1 needed medical attention. Facility contacted the parent for C1 to be picked up. Facility representative stated that C1 returned to school the next day and resumed normal activities. During today\342\200\231s inspection, the facility was toured and the outdoor play equipment was inspected and photographed. LPA reviewed staffing and class ratio and discovered that the facility was operating within ratio requirements. There were 15 children with three staff members when the incident occurred. LPA received a copy of the incident report that was provided to the parent. LPA interviewed one staff member (S1) who stated they were actively supervising C1 while they were injured. S1 said C1 was twirling on the play structure and the next moment they saw C1 was on the step holding their eye. S1 had requested that C1 slow down with the twirling prior to the injury. S1 recognized C1 was bleeding and immediately removed C1 from the playground to provide first aid. Facility representative called C1's parent to be taken to the physician. S1 said the injury happened so quickly they did not believe anything could have been done to prevent the incident from happening. LPA received contact information for C1's parent and contact information for two staff members who were also on the playground during the incident. Due to insufficient information at this time further investigation is needed. Exit interview conducted and report was reviewed with the facility representative Amy Beth Ahl-Wright. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
