What the inspector wrote

On 10/15/24 at 2:30pm, Licensing Program Analyst (LPA) Elizabeth Friese conducted an unannounced case management inspection and met with facility representative Kaitlin Vondran due to an unusual incident which was self-reported to the Department on 10/01/24. On 10/01/24, at approximately 9:50am, a child had gotten their leg stuck in a play structure which required EMT response to extricate them. Parents were notified immediately and declined further medical treatment. During the visit LPA Friese and the Director toured and inspected the play structure. It was determined to be age appropriate and being used to manufacturer’s guidelines. The incident was most likely a fluke that would not occur again. This report was reviewed with the Director Kaitlin Vondran and appeal rights provided. Notice of site visit to be posted for 30 days.
