Inspector’s narrative

What the inspector wrote

S3 stated it was true they would prepare breakfast and did not have 100% supervision of children. S3 stated this lack of supervision has been addressed since the complaint was opened and now, they bring in another staff member to prepare meals so S3 can maintain 100% supervision of children in care. Four parents were interviewed on 11/26/24 and 3/5/25 and two parents (P3,P4) indicated they have seen S3 preparing meals in the kitchen without a direct line of sight to children in care. Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), CCR 101229(a)(1) is being cited on the attached LIC 9099D. Exit interview conducted and report was reviewed with the facility representative. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

    (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This was not met as evidenced by: based on interviews it was determined that children were left in the preschool room without 100% supervision while staff prepared meals. This poses a potential health, safety, or personal rights risk to persons in care.
