What the inspector wrote

An unannounced case management inspection was conducted today at 3:05pm by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with facility representative Chelsey Bleeke in response to an Unusual Incident Report received by the Department on 8/20/25. A child in care, C1 was injured outdoors near the picnic table and required medical treatment. C1 dislocated their knee during the incident. The facility representative was interviewed on 9/3/25 at 3:05pm and stated that on 8/19/25 at approximately 3:15pm a child in care (C1) injured their knee on or near the picnic table and caused a dislocation of the knee bone. C1 was taken by ambulance to the emergency room and it was determined the knee was dislocated. One staff was interviewed on 9/3/25 and stated that C1 tweaked knee near the picnic table and emergency services were notified. C1 showed visible signs of dislocation and parent was notified immediately. Staff indicated they did not feel anything could have been done to prevent the accident. Parent was interviewed on 9/2/25 and stated they believed the injury was an accident and they were not concerned about a lack of supervision. C1 was interviewed and re-enacted the incident for LPA. LPA asked how the incident occurred and C1 said they didn't know how it happened but they think they twisted their leg and tweaked the knee. When asked if they thought the injury could have been prevented, C1 said they should walk away from the table before twisting their leg. During today’s inspection, the facility was toured and 30 children were present with 5 staff members. LPA observed the outdoor play area and assessed the picnic table which was deemed age appropriate for the children in care. Based on the interviews conducted and ratio requirements being met LPA determined this incident was an accident. No further investigation is needed. Report was reviewed and discussed with the Director, Chelsey Bleeke. All licensing reports are public information and must be made available upon request for at least three years. Notice of Site Visit shall be posted for 30 days from today's visit. Appeal rights were provided.
