What the inspector wrote

On 9/29/25 an informal office meeting was conducted at the facility by the Regional Manager, Jordan Monath, Licensing Program Manager, Erin Virrueta, Licensing Program Analyst, Tammy Dutra and Licensee, Dionna Lefkowitz. The purpose of this meeting was to discuss a Type A deficiency delivered on 9/23/25 regarding multiple incidents where children in care have been injured on the mountain of energy. The facility was toured and LPA, LPM and RM reviewed the mountain of energy and took pictures of the facility. We reviewed the facilities policy with regards to the mountain and requested a copy. We discussed the placement of staff during play time on the mountain and reviewed activities children are allowed to participate in while in play. We watched children's activity on the mountain. Licensee has clear understanding of reporting requirements. Licensee has posted reminders for staff regarding reporting requirements and safety while at the facility. Licensee understands the mountain of energy policies must be enforced. Report reviewed with the Licensee, Dionna Lefkowitz. Appeal rights provided. Notice of Site Visit was given and must remain posted. Failure to post will result in $100 Civil penalty.
