What the inspector wrote

On 4/16/24 at 3:06pm Licensing Program Analyst (LPA) Bianca Mendez conducted a unannounced case management visit for the purpose of lead testing. LPA Bianca Mendez met with licensee/director Neysa Neely. LPA had verified that facility did not use a certified external water sampler. During today's visit LPA observed 14 children in care. The following deficiency is being cited (see LIC 809D). A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee Neysa Neely.
