What the inspector wrote

On 4/25/25 @ 11:59am an unannounced case management inspection was conducted by Licensing Program Analyst (LPA), Erica Laird. LPA met with licensee, Ivory Gross. In response to an Unusual Incident Report received by the Department on 4/10/25. On 4/09/25 two children (C1 and C2) were playing outside and collided causing C1 to fall on their left leg. C1 was unable to put weight on their leg. Parents were called and C1 was taken to urgent care. It was later determined C1 had sustained a hairline fracture to the left leg. The licensee, Ivory Gross, was interviewed on 4/25/25 at 12:14pm and stated that on 4/9/2025 kids were running outside C2 ran into C1 causing C1 to fall onto their left leg. C1 was not wanting to put pressure on their left leg. Parents were called and C1 was taken to urgent care. It was determined C1 had sustained a fracture on the leg. Ivory stated C1 was in a boot until yesterday. During today's inspection LPA Laird conducted one staff interview (S1). S1 stated they were outside on the playground with the kids when they heard C1 crying. S1 saw C1 on laying on their side crying. S1 picked C1 up and noticed C1 was not wanting to put weight on their leg . S1 took C1 over to Ivory and Ivory called C1's parents. During today\342\200\231s inspection, the facility was toured . LPA Laird inspected the playground and did not observe any hazards. Based on interviews and observations, it does not appear the injuries sustained were due to lack of supervision or hazards. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the licensee, Ivory Gross . Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
