What the inspector wrote

On 5/1/26 @ 4:14pm Licensing Program Analyst (LPA) Erica Laird conducted an unannounced case management inspection and met with facility director Linda Wright. During today's inspection the facility was toured. During a previous inspection of the facility on 4/2/26 LPA Laird determined the facility had undergone construction to the front room. LPA Laird spoke to facility director Linda Wright who confirmed the facility did not notify licensing of the construction. On 4/2/26 LPA Laird met with owners Sherrie and Steve Allard. Sherrie and Steve confirmed that the facility had failed to notify licensing of the construction. Based on observations and interviews with facility director and facility owners, the following deficiency is being cited on the attached LIC809-D: 101237(a) Alterations to Existing Buildings. Exit interview conducted and report was reviewed with facility director, Linda Wright. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
