What the inspector wrote

On 9/23/25 an informal office meeting was conducted via Microsoft Teams by Licensing Program Manager, Megan Aviles, Licensing Program Analyst, Erica Laird and Licensee, Heather Lingemann. The purpose of this meeting was to discuss two Type A deficiencies, and one Type B deficiency cited at the facility on 8/28/25. On 8/28/25 the facility received an A citation for lack of supervision. Specifically, a child was left unattended on the playground for approximately 48 seconds. Licensee understands children must have constant supervision. Licensee stated, as of 9/23/25, all staff have been trained on active supervision. On 8/28/25 the facility received an A citation for children having access to hazardous materials. Specifically, two children were exposed to a bleach/water mixture. One child spilled the bleach mixture on their skin and one child sprayed their face with the bleach mixture. Licensee understands the seriousness of keeping hazards inaccessible to children. Licensee stated all staff are aware of this requirement and will ensure hazardous materials are kept inaccessible to children. On 8/28/25 the facility received a B citation for not following reporting requirements. Specifically, the facility failed to report the lack of supervision and bleach exposure to the Department as required. The facility did not report incidents to parents. Licensee has clear understanding of reporting requirements. Licensee has posted reminders for staff regarding reporting requirements and time frames. The Department received the licensee’s written Plan of Correction on 08/29/2025 for all deficiencies cited on 8/28/25. All deficiencies cited on 8/28/25 have been cleared. This report was read and reviewed with the licensee.
