Inspector’s narrative

What the inspector wrote

On 8/10/2021 at 11:00am, Licensing Program Analyst (LPA) Kirk Marks met with the Lead Teacher, Lexi Ramsey, for the purpose of delivering findings of a complaint investigation completed by Chris Krogstad, Special Investigator for the Department of Social Services Bureau of Investigations (IB). It was alleged that a child sustained a concussion while in care due to a lack of supervision. Two staff members were interviewed and both stated that on 4/08/2021 the child was observed to bump their head on the edge of a bookshelf while playing, causing a small red mark near the child’s eye. Both staff members reported that the child did not cry when this occurred and was treated with a cold pack. The facility reported the injury to the child’s parent. Through the interviews that were conducted and observations, the Department could not determine that a child was injured due to a lack of supervision at the facility. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the allegation violation occurred, and the findings are unsubstantiated. An exit interview was conducted. The Notice of Site Visit must be posted for 30 days.
