Five Staff (S1-S5) were interviewed on 10/25/23 and denied the allegations. S1 \342\200\223 S5 stated that C1\342\200\231s care was terminated because P5 was upset that there was not a spot in the preschool room for C1 and P5 became rude and non-compliant with staff. S1 \342\200\223 S5 stated that P5 would not bring the proper supplies such as diapers and wipes to the facility for C1 as well. S1 \342\200\223 S5 denied any knowledge of retaliation. Four Parents were interviewed on 12/08/2023 and denied the allegations. P1 \342\200\223 P4 were unaware of the allegations and had no knowledge of any child\342\200\231s care being terminated out of retaliation. During today\342\200\231s visit facility was toured. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. Exit interview conducted and report was reviewed with the facility representative Shelly Tellez. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
