What the inspector wrote

LPA Mendez interviewed seven staff (S1-S8) on 3/7/22 3/8/22, 3/10/22/, 3/11/22, and 3/29/22. LPA Mendez asked staff if they were changing children’s diapers or pull ups. Four out of eight staff members stated no, regarding staff changing children’s diapers. S1 stated that there was no consistent schedule for diaper changes and there were only 4 scheduled diaper changes throughout the entire day. S2 stated that staff were not changing children’s diapers and did not have a log. S6 stated staff were not changing children’s diapers. S7 stated that staff did not change children’s diapers as needed and throughout the entire day children were changed 4 times a day. LPA Mendez interviewed 19 parents on 3/2/22, 3/7/22 3/14/22, 3/15/22, 3/17/22, 3/24/22,3/28/22 and 3/29/22. LPA Mendez asked parents if staff had been changing children’s diapers. Five out of the 18 parents confirmed that their child does not wear diapers. LPA Mendez asked parents if they had ever received a diaper log from the facility. It was 15 out of 19 parents that confirmed that they do not receive diaper logs. LPA Mendez interviewed seven staff (S1-S8) on 3/7/22, 3/8/22, 3/10/22, 3/11/22, 3/29/22 and 4/12/22. LPA Mendez interviewed staff and asked if the facility was operating out of ratio, 3 out of 8 staff had confirmed that the facility was operating out of ratio. LPA Mendez interviewed 19 parents on 3/2/22, 3/7/22 3/14/22, 3/15/22, 3/17/22, 3/24/22,3/28/222 and 3/29/22. LPA Mendez asked parents if facility was operating out of ratio 13 out of 19 parents had witnessed facility operating out of ratio and staff shortages. LPA Mendez observed that there was an erasable potty chart and that there was no diaper log being sent home to parents whose children wear diapers or pull ups. Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D. Appeal rights were provided and exit interview conducted. The Notice of Site Visit must be posted for 30 days.
