On 11/07/25 @ 8:40am an unannounced case management inspection was conducted by Licensing Program Analyst (LPA), Erica Laird. LPA met with Shelly Caperello in response to an Unusual Incident Report received by the Department on 11/04/25. Per the incident report, a child (C1) placed an activity cube on their head and got stuck. Staff removed the cube which caused minor red marks on C1's head. On 11/07/25 LPA Laird conducted an interview with owner Shelly Caperello . Shelly stated she was asked to come into the classroom because C1 had placed an activity cube on their head and it was stuck. Shelly stated she held C1 while S3 helped her to remove the cube. Shelly stated they were about to call the fire dept. but decided to try using oil to remove the cube. Shelly stated with the use of olive oil the cube was able to come off. Shelly stated C1 had a couple of red marks on their head from removing the cube. Shelly stated she immediately called C1's parents and C1 was picked up. Shelly stated C1's parents have disenrolled C1 from care. During today’s inspection, the facility was toured . LPA Laird inspected the infant classroom and infant play yard. LPA Laird observed the play yard to be in the middle of the classroom with direct visibility. LPA Laird observed the play cube did not have an age recommendation on it. LPA Laird took 4 photos, obtained a photo of C1 with marks on their head, documentation of communication and infant sign in sheet from day of incident. LPA Laird conducted interviews with staff (S1-S4). All staff interviewed stated no one observed C1 place the cube on their head. Based on interviews, there were 9 infants and 2 staff present at the time of incident. Based on interviews and observations, the following deficiencies are being cited on the LIC809-D. 101439(I)(4)(b) Infant Care Center Fixtures, Furniture, Equipment and Supplies; 101429(a)(1)Responsibility for Providing Care and Supervision for Infants; 101416.5(a) Infant Ratio Exit interview conducted and report was reviewed with owner Shelly Caperello. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
