Correction check·January 14, 2026

On January 14, 2026, at 2:0 0PMLicensing Program Analyst (LPA) Deborah Khashe and Licensing Program Manager (LPM) Bettina Engelman met with facility representative Sarah Kelly to initiating a Proof of Correction inspection. LPA Khashe explained the purpose of the visit to Facility Representative. LPA and LPM toured the facility during the inspection and made observations. There were seven (8) children and three (3) staff present.The purpose of today's visit was to clear the deficiencies received on December 12, 2025. LPA and LPM reviewed files and files are still missing required documents health screens and immunization records. Civil Penalty is assessed. An Exit Interview was conducted in which the report was reviewed and discussed with Facility Representative, Sarah Kelly. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights provided.
