Routine inspection (3-year)·December 12, 2025

On December 12, 2025, Licensing Program Analysts (LPAs) Deborah Khashe and Katy Valazquez met with facility representative Nicole Thomasy and Emma Arvin for the purpose of an unannounced annual inspection. LPAs explained the purpose of the inspection and entrance was granted. Facility days and hours of operation are Monday through Friday from 7:00 AM to 6:00 PM. There were (21) children present during today's inspection. LPA discussed with facility representative the annual license fees and provided the facility representative the PIN number and the balance of the annual license fees for online payment. LPA reviewed care and supervision of children, staffing ratios, medications and first aid supplies, furniture, equipment, fire drills and drinking water. There are adequate toys and equipment available for children. Outdoor play area was toured, the play structure appeared to be in good repair, and there is sufficient cushioning (artificial grass) under the play structure to prevent the fall. LPA observed all required forms to be posted. LPA reviewed the sign in/out app (Bright Wheel) and observed that the children are properly signed in. LPA observed health screening reports and requested proof of immunization's. LPA observed AB1207 mandated reporter training for some staff present at the time of the inspection. The facility representative was reminded to renew the course every 2 years through www.mandatedreporterca.com website. Report continues on LIC809-C... (Page 2) Page 2 LPA reviewed children\342\200\231s and staff files. All staff present during today's inspection have a fingerprint clearance and associated to the license. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Facility Representative understands that if an unusual incident occurs, licensing is to be notified via phone call, e-mail or fax within 24 hours, and the Unusual Incident Report LIC 624 shall be submitted within 7 days to remain in compliance. Applicant understands that if any structural changes are made to the facility, licensing must be notified prior to construction. Facility Representative understands that if they want to make any off-limit area an ON-limits area, they must notify licensing, and LPA must do an inspection BEFORE children are allowed in the area. Facilty Representative understands that children\342\200\231s records are to be maintained according to Title 22 regulations and be accessible to licensing for up to three years. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. In the areas that were evaluated, 2 type A and 1 type B deficiencies were cited during the inspection. LPA informed staff member Emma Arvin that this report, dated 12/12/25, documents 2 Type A citations which shall be posted for 30 consecutive days as there is immediate risk to the health, safety , or personal right to children in care. Also, LPAs informed Emma Arvin to provide a copy of this licensing report dated 12/12/25 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed acknowledgement of receipt of licensing report, LIC 9224, or other written statement, must be placed in the child's file for verification.
