Follow-up on corrections·August 20, 2025

An unannounced case management inspection was conducted today at 2:10pm by Licensing Program Analyst (LPA), Laura Chavez. LPA met with licensee Angelica Rivera. The inspection is a follow-up to the inspection made on 7/10/2025, at which time the licensee\342\200\231s assistant (A1) was unable to provide proof of their identity. The licensee was also unable to provide proof of A1's identity after she could not gain entry into the bedroom, where she maintains records on her assistant after a child jammed the keyed locking doorknob. On 7/22/2025, the licensee provided a copy of A1\342\200\231s driver's license (DL); however, the name given by A1 on 7/10/2025 did not match the name on the DL provided. The following deficiency was cited: 102370(d)(1) Criminal Record Clearance (see LIC 809D). LPA Laura Chavez informed licensee Angelica Rivera that this report, dated 8/20/2025, documents one Type A citation. Type A citation(s), which shall be posted for 30 consecutive days, as there is an immediate risk to the health, safety, or personal rights of children in care. LPA Laura Chavez informed the licensee to provide a copy of this licensing report dated 4/23/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted, and the report was reviewed with the licensee, Angelica Rivera. A plan of correction was discussed. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
