What the inspector wrote

On 10/29/2025 at 2:00 PM Licensing Program Analyst (LPA), Lorraine Dacanay Breaux conducted an unannounced annual/random inspection. LPA met with Director, Maggie Kwong. LPA provided the Entrance Checklist (LIC 125). Present during today's inspection was 6 staff (includes administrative staff) and 20 preschool age children in care. The facility operates Monday \342\200\223 Friday from 8:15 AM \342\200\223 5:30 PM. (08:15 AM - 2:00 PM school hours). The facility was toured with the director for health and safety inspection. Classrooms: Two classrooms are utilized for this license. Both classrooms were inspected for age-appropriate furnishings, equipment, & adequate storage for children\342\200\231s belongings. LPA observed the cleanliness of floors & surfaces, the presence of a fully functional carbon monoxide detector, smoke detector, pull down fire alarms, and a fully charged 3A40BC fire extinguisher that is accessible throughout the facility. The center is equipped with a fully stocked first-aid kit and available in the classrooms. Per director there are no fire arms on the site. Food Service Areas: The facility only provides snacks. There is a designated area, were the supplies are kept. All center provided food items are properly labeled & stored separately from cleaning supplies. Food prep area is clean, adequately equipped, & free from hazardous materials. Snack menu is posted one week in advance, available for review, & dated. LPA advised director that food is to be supplied if a child is still hungry after lunch/snack. Bathrooms: Facility has separate staff and child designated bathrooms. Toilets and faucets are in safe and sanitary operating condition. The children are able to reach the sinks and toilets. Supplies are available. Page 1 of 3 ***Continued on LIC 809C*** Outdoor Play Area: The outdoor playground was observed to be fully fenced and free from defects or hazardous conditions. The area includes large trees that provide shade for children while at play, as well as playground equipment designed for preschool age children. The playground features a swing set anchored to the ground with cushioning material placed underneath to absorb falls. A climbing and slide structure was also observed with appropriate cushioning to prevent injury. All play equipment was age-appropriate and in good repair. One shed used to store playground equipment/school supplies, located in the yard. There are no pools/bodies of water at the facility, director confirmed there are no pools/bodies of water at the facility. Records: At 2:30PM LPA requested and reviewed children and staff files. LPA reviewed six (6) children\342\200\231s files and six (6) staff files. LPA reviewed the facility roster & personnel report. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Mandated Reporter certificates were reviewed. Director was reminded of their responsibility to renew CPR/First Aid and Mandated Reporter certificates every two years. The center is in compliance with sign in and out procedure. Emergency Drills are recorded and performed at least every six months. All required documents are posted in a publicly accessible area. Criminal Record Clearance : Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Lead Testing: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Lead Report is posted on the parent board. TRANSPORTATION: LPA inspected the van used for transporting children. LPA reviewed the Driver's License for the driver and is valid. Director is reminded to never eave a child in the vehicle unsupervised. Director is reminded to have to proper seat belts/car seats for children. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP . Unusual Incident/Student Injury Report: Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child\342\200\231s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days. MyChildCarePlan.org: Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
