What the inspector wrote

On 05/14/2025 at 8:55AM, Licensing Program Analyst (LPA) Jaleesa Jackson conducted an Unannounced Case Management inspection for an incident that happened on 5/6/2025. LPA met with Director Jayne Martin. Also present during the inspection were 19 fingerprint cleared staff and 100 preschool aged children in care. The incident occurred on 05/06/2025 and was self-reported to Licensing on 05/07/2025. The director stated that S1 was trying to get C1 to come over to them because they were concerned that C1 was going to fall and C1 did not want to. C1 began to pull away from S1. C1 began to cry and stated their arm hurt. C1 sustained a dislocated elbow from the incident LPA reviewed records and interviewed staff about the incident. There was 1 deficiency cited on today's visit. See 809-D for deficiency. LPA Jackson informed Facility Representative Jayne Martin that this report dated 5/14/2025 document 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Jackson informed the facility representative to provide a copy of this licensing report dated 5/14/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Continued on 809-C A notice of site visit was given and must remain posted for 30 days. Appeal Rights were given and discussed. An exit interview was conducted

    Upon the occurrence, during the operation of the child care center of any of the events ... a report shall be made to the Department by telephone or fax within the Department's next working day... Events reported shall include the following: Any injury to any child that requires medical treatment. This requirement is not met as evidenced by:Based on interview and record review the school was notified 2/12/25 that C1 recieved medical treatment but did not inform licensing until 2/18/25, which poses a potential health and safety risk to children in care.

  • PERSONAL RIGHTS

    The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature...This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the section cited above in when C1 was pulling away from S1 his elbow was dislocated which poses/posed a immediate health, safety or personal rights risk to persons in care.
