What Happened

Unannounced case management inspection following self-reported incident on 5/6/2025 where child sustained dislocated elbow. Director stated staff member was concerned child would fall and child pulled away during interaction. One Type A citation issued.

Observations

  • Child C1 sustained dislocated elbow from incident
  • Incident occurred when staff member S1 tried to prevent child from falling; child resisted and pulled away
  • Child began crying and reported arm pain
  • Type A citation issued
