What Happened
Critical supervision failure - child left premises unattended for 20 minutes when utility workers unlocked perimeter gate and staff congregated. Child attempted to walk home and was retrieved by neighbor. Type A violation issued.
Observations
- child (C1) left premises unattended through unlocked gate
- child missing for approximately 20 minutes
- utility workers unlocked gate without staff knowledge or permission
- gate intended to remain locked was unsecured
- all staff congregated together for several minutes, leaving children unattended
- child attempted to walk home
- child retrieved and returned by good samaritan neighbor
