What Happened
Child left facility premises unattended for 15 minutes; this is the second incident within months. Type A deficiency cited with repeat violation penalties and $250 civil penalty.
Observations
- On 12/16/22, child C1 left premises unattended while under S1's supervision
- S1 gave C1 permission to use hallway restroom; C1 ran through hallway into empty room instead, pushed front door open, and exited premises
- Incident occurred at 12:30 PM during lunch time with CCTV footage available
- Child C1 found by neighbor approximately 15 minutes later
- This is second incident with same child—previous incident one month earlier (11/16/22) when child left unnoticed and found by same neighbor
- Facility previously cited Type A on 11/16/22 with $500 civil penalty
