What the inspector wrote

On 10/22/2025 at 3PM Licensing Program Analyst (LPA) Kareeca "Reeca" Sykes met with the Principle of the Holy Rosary Elementary School Melissa Ortiz - Gray which is attached to the Holy Rosary Preschool facility (Due to the facility currently not having a Director). During an initial visit on 10/08/2025, where LPA attempted to conduct an Annual Inspection, LPA was denied entry by the Principle of Elementary School due to the facility being on "lock down" for a potential threat at the facilities church which is located in a building outside of facility to the right. LPA instructed the Principle to call the Community Care Licensing Regional Office to submit an Unusual Incident Report (UIR) as required. On a follow up visit to conduct the Annual Inspection on 10/22/2025, it was determined that the facility did not submit required UIR. During this inspection LPA provided extensive training on reporting timelines (24 hour phone notice and 7 day written submission) and instructed the Principle to submit the missing UIR immediately. SEE LIC 809-D FOR DEFICIENCIES Exit interview conducted and report was reviewed with the Holy Rosary Elementary School Principle, Melissa Ortiz -Gray. A Notice of Site Visit was given and must remain posted for 30 days.
