Incident investigation·May 4, 2026

On 5/4/2026 at 9:46am Licensing Program Analyst (LPA) Morgan Pringle met with facility staff for an unannounced case management visit for an incident that was self reported by the facility on 4/10/2026. Present during the visit were two (2) staff members. The Director, Dana Shichida arrived around 10:15am. There were no children at LPAs arrival and thirty-five (35) children arrived at 11:30am The facility is located on the far right side of the Sycamore Valley Elementary School campus and operates in one building, in three (3) rooms. The facility operates from 7:00am - 6:30pm, Monday - Friday. On 4/9/2026 it was stated a child's personal rights had been violated. During transition and clean up time a teacher (T1) was seen yelling at a child (C1) and grabbed the child's arm. After that, T1 was sitting at the carpet with a group of children and asked the children to raise their hands if they though C1 had been "bad," which caused the children to laugh and taunt C1. Another teacher stepped in and redirected the children and told them that was not ok to say or do to someone. Staff reported the incident to Director who was not present at the time and T1 was instructed to leave later that day. An internal investigation was conducted by Kids Country and T1 was terminated on 4/17/2026. A type B deficiency is being cited today for the violation of C1's personal rights (See LIC809D). A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Dana Shichida.
