What the inspector wrote

On 09/23/2025 at 03:00 PM, Licensing Program Analyst (LPA) Kareeca "Reeca" Sykes conducted a Case Management Inspection at La Petite Academy Inc. LPA met with Director, Jamie Czelusniak and explained the purpose of this visit. During today's inspection, there are 32 preschool children in care with (4) four staff in 3 (three) classrooms. Director stated there are 45 preschool children enrolled. LPA was following up on an Unusual Incident Report which was reported on 09/18/2025. During the visit, an LPA and the Director attended a teams meeting with Kierstin Bulkner (The facilities district manager) who was able to access the facilities camera footage. During the meeting LPA observed video footage of S2 pulling C1 down to the cot by C1's ankle and then pushing on C1's should to lay them down on the cot. California code of regulations, Title 22 states each child is accorded the following personal rights ... To be free from...unusual punishment...or other actions of a punitive nature. The facility is out of compliance with Title 22 regulations. LPA Reeca Sykes informed the Director, Jamie Czelusniak that this report dated 09/23/2025 documents a Type B citation. Type B citation(s) are a potential risk(s) to the health, safety, or personal rights of children in care. *SEE LIC 809-D FOR DEFICIENCIES* Exit interview conducted and report was reviewed with the Director, Jamie Czelusniak. Notice of Site Visit was given and must remain posted for 30 days.
