What the inspector wrote

On 12/18/2025 at 9:25 AM, Licensing Program Analyst (LPA) Christina Watts conducted an Case Management Inspection at Contra Costa County Head Start - Balboa CDC. LPA met with Site Supervisor III, Marilyn Doss and explained the purpose of today's inspection. During today's inspection, there were 71 preschool children in care with 26 staff in six classrooms. Site Supervisor III stated there are 82 preschool children enrolled. LPA is returning to following up on an self reported Unusual Incident submitted to licensing on 10/15/2025. LPA toured the facility, conducted interviews and obtained relevant documents. Per staff interviews, on 10/14/2025 in the morning, a parent and child came inside Classroom 3 with outside food. Per interviews, outside food is not allowed inside the classrooms. Interviews stated staff informed the parent that if they want the child to eat the food, they would have to go outside the classroom and have the child eat the food. Interviews stated that C1 was trying to go outside with the parent and the parent had told C1 to stay inside. Per interviews, once the parent went outside with their child, C1 followed behind the parent and went to the play yard. S1 stated they seen C1 at the same time the Site Supervisor seen C1 and returned C1 to their classroom. Per staff interviews, they stated C1 was left alone for about 2-15 minutes. Site Supervisor stated they reviewed video and C1 was left unsupervised for no longer than 1 minute. Per California code of regulations, Title 22 stated children are not to be left unsupervised at any time. This facility is out of compliance with California code of regulations, Title 22. LPA Christina Watts informed Site Supervisor III, Marilyn Doss that this report dated 12/18/2025 documents a Type B citation. Type B citation(s) are a potential risk(s) to the health, safety, or personal rights of children in care. *SEE LIC 809-D FOR DEFICIENCIES* Exit interview conducted and report was reviewed with the Site Supervisior III, Marilyn Doss. A Notice of Site Visit was given and must remain posted for 30 consecutive days.
