On 11/24/2025 at approximately 02:30pm, Licensing Program Analyst (LPA) Jamel Maiwandi conducted an unannounced annual inspection and met with assisting site supervisor Lisa Tuimavave and head teacher Lorena Perea and explained the purpose of the visit. Hours of operation are from 7:00am-5:30pm Monday to Friday. There is one preschool classroom located in the facility with 6 preschool children and 2 staff present. Total preschool children enrolled are 20 as per head teacher. All staff members present received criminal record fingerprint clearance. The facility is within its capacity limits and ratios for today\342\200\231s visit. LPA observed that all required documents, such as the facility license, notification of parents\342\200\231 rights, personal rights, car seat laws, emergency disaster plan, and daily activities schedule, were displayed and visible to the public. The most recent emergency disaster drill was conducted on 10/16/2025, and LPA observed that it was documented. LPA reminded director that emergency disaster drills must be conducted every 6 months. The facility was observed to have one fully stocked first aid kit in the classroom, a functioning carbon monoxide detector, and one fully charged 3A40BC fire extinguisher located in the classroom as well. LPA inspected the indoor and outdoor preschool area to identify health and safety hazards. The facility was clean, safe, sanitary, and in good repair. Disinfectants, cleaning solutions, poisons, and other items that could pose a danger were stored in areas inaccessible to the children and located in each classroom. All accessible cabinets and drawers in the classrooms were found to be free of hazardous materials or poisons. The facility does administer prescribed medications and LPA verified medications are current and unexpired. Continues on 809-C Page 2 LPA observed the preschool classroom to have age-appropriate toys, furniture, educational materials, and designated storage space for each child. Electrical outlets were fitted with child safety covers to prevent children's access. There are two toilets and two sinks available for children in the classroom and LPA observed bathrooms to be clean, fully operational, and equipped with suitable toileting equipment and sanitation products. The facility currently provides breakfast, lunch, and a PM snack and menu items are displayed in the facility. The outdoor playground has a fence that is at least four feet tall. The play equipment is in good condition and does not have sharp, loose, or pointed parts. The areas around and under climbing equipment, swings, and slides are cushioned with resilient padding. There are no pools, spas, or bodies of water located on the property. Site supervisor was reminded to inspect daily for hazards that could pose a health and safety risk to children. LPA reviewed the files of five preschool children and verified that their emergency contact and medical information were complete. LPA reviewed five staff files and confirmed that all required forms were present. LPA reminded site supervisor to ensure staff have updated qualifications and trainings at all times. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Site supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
