On 04/17/2025 at 1:25PM LPA's Kareeca "Reeca" Sykes and Ashley Hollinger arrived at the facility and met with Licensee Yolanda Evans for a Case Management Inspection. Present during the inspection was the licensee and two (2) assistants who are fingerprint cleared. LPA's observed 14 daycare children (2 infants 9 preschool and 3 school-age). The purpose of this inspection is to serve an Immediate Exclusion Order for Dasjha Jones. During today\342\200\231s inspection the Order of Immediate Exclusion has been provided, explained and discussed with the licensee. Licensee states she understands that Dasjha Jones cannot be physically present at the facility. Licensee was given form LIC 995B \342\200\223 Family Child Care Home Addendum to Notification of Parents Rights (regarding Removal/Exclusion). Licensee was informed that parents of children in care and parents of any newly enrolled clients will need to receive this form. An immediate civil penalty will be cited for failure to provide a copy of this form to one parent or authorized representative of every child in care; and a copy to one parent or authorized representative of every new child enrolled as long as the exclusion is in effect. Copies of the signed forms must be kept in each child\342\200\231s file. LPA's will review these forms at each facility inspection. A copy of the exclusion letter/order was also left at the facility for Dasjha Jones. A copy was mailed on 9/21/24. Licensee was also given form LIC 995D \342\200\223 Family Child Care Home \342\200\223 Explanation of Removals/Exclusions and Reinstatement. During today's visit, LPA's received a copy of the facility roster. Licensee stated they will submit proof of LIC 995B by 04/25/2025. Exit interview conducted with Licensee Yolanda Evans.
