What the inspector wrote

Licensing Program Analyst (LPA) Canela arrived unannounced on 10/22/2025 at Kindercare Learning Center - Infant Center. The purpose of this visit is to conduct a case management - deficiencies inspection after deficiencies were observed during a separate inspection relating to a complaint. The Infant Center component of this facility currently has 90 children enrolled. Present during today's inspection were 2 teachers in the toddler room with 5 toddlers, and 2 teachers in the infant classroom with 4 infants. Community Care Licensing, Oakland Child Care Regional Office became aware of unusual Incidents which occurred in 2025 during the months of April, June, July, and October 6th, 2025. Confirmed cases and cases of children exposed to hand, foot, and mouth were determined in the infant and toddler's classroom during those months. Interviews conducted and record review revealed Kindercare Learning Center did not report the incidents to the State of California, Department of Social Services, Community Care Licensing (CCL) as required. LPA confirmed Kindercare did not submit an incident report or notify CCL by phone or fax. This is a potential risk to the health, safety, and personal rights of children in care. report continued on LIC809-C Page 2 During record review, LPA requested a copy of a current and complete Child Care Facility Roster during an inspection on 10/21/2025. Kindercare did not provide a copy and LPA was informed "they did not have it". LPA requested the Child Care Facility Roster be completed. On 10/22/2025, Assistant Director Pam provided LPA with a copy of the Child Care Facility Roster. However the Child Care Facility Roster was not complete as the Assistant Director stated they did not know how to add the dates children were enrolled and the dates the children were disenrolled/left the facility. The Child Care Facility Roster dated 10/22/2025 only has the names and info of children currently enrolled. The Child Care Facility Roster is missing: 1. The enrollment dates the children 2. The names of children and their info who are no longer enrolled 3. The dates when the children who are no longer enrolled left the facility. Additionally LPA discussed this requirement with Assistant Director during the annual inspection on 07/16/2025. LPA did not cite during that time as Assistant Director stated they would complete and be in future compliance. Deficiencies cited (see LIC 809-D page) from the California Code of Regulations, Title 22, Division 12, Chapter 1. Failure to correct the deficiency and/or repeat deficiencies within a 12 - month period may result in civil penalties. Appeal Rights Provided. Exit interview conducted with Director Shawn Powers, whose signature confirms receipt. A notice of site visit was given and must remain posted for 30 consecutive days.

This requirement is not met as evidenced by: Based on interview and record review, the licensee did not comply with the section cited above due to not providing Community Care Licensing (CCL) with the Children's Roster (LIC 9040) as required and stated by assistant Director that there was no Roster available to provide to CCL.
